NOTICE TO PATIENTS


WE ARE AWARE THAT THERE IS AN INTERMITTENT PROBLEM WITH OUR TELEPHONE SYSTEM.


THE PROBLEM OCCURS AT BUSY TIMES AND RESULTS IN CALLERS BEING CUT OFF WHILST WAITING FOR THEIR CALL TO BE ANSWERED.


THE PROBLEM HAS BEEN REPORTED TO OUR TELEPHONE PROVIDER WHO IS TRYING TO RESOLVE THE PROBLEM.


WE APPRECIATE THAT WHEN THIS HAPPENS IT CAUSES FRUSTRATION FOR WHICH WE SINCERELY APOLOGISE.


WE ARE PRESENTLY LOOKING AT ALTERNATIVE TELEPHONE PROVIDERS SO THAT WE SOURCE A MORE ROBUST SYSTEM

Any patient requesting On Line Access will need to complete an On Line Access Registration Form that will need to be completed and submitted to the surgery in person together with one photographic form of ID (e.g. passport or driving licence) and one proof of address that is no more than 3 months old.

Detailed Coded Records

Patients who have access to on line services can have the facility to view their detailed coded online record and they can apply for this via their on line access. This includes all information that is held on the practice's clinical system which is held in coded form unless any of the following apply:

In the reasonable opinion of the practice, access to such information would not be in the patient's best interest because it is likely to cause serious harm to:-

  • The patient's physical or mental health or
  • The physical or mental health of any other person
