NOTICE TO PATIENTS REGARDING CORONAVIRUS

We are aware many patients are anxious about the change in guidance from July 19th, and whether they should be wearing a face mask or not. We would like to commit to our continued efforts to safeguard our staff and patients and will be asking for the continued use of masks in all environments at the Practice, including the waiting room, reception, clinical spaces and administrative spaces. May we thank you in advance for your co-operation .

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore athealth_clipboardtract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Fitness to Fly letter
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Forms for Signature

We respectfully ask patients requiring this service to leave full instructions as to the completion of the form and whether they will be collecting it or prefer for it to be posted on completion. The doctors provide this service privately and there will be a fee charged for each service provided. We cannot guarantee a time for form completion, as this has to be fitted in with their usual duties. Patients requiring their completed forms posted to the relevant address must ensure that their form is accompanied by a stamped addressed envelope. Forms not having an envelope will be left at reception for the patient to collect.
