THERE IS NO CAR PARKING ON SITE AT BURBAGE SURGERY DUE TO THE  BUILDING DEVELOPMENT. ALTERNATIVE PARKING IS AVAILABLE ON THE CAR PARK OFF FEATHERSTONE DRIVE.

PLEASE AVOID PARKING NEAR TO THE ENTRANCE TO THE SURGERY AS THIS IS HAZARDOUS

Please do not book a GP appointment or attend your GP Practice if: 

  • you think you might have coronavirus

  • in the last 14 days you've been to a country or area with a high risk of coronavirus – see our coronavirus advice for travellers

  • you've been in close contact with someone with coronavirus

  • NHS 111 has an online coronavirus service that can tell you if you need medical help and advise you what to do. 

  •  Call 111 if you need to speak to someone. 

Any patient requesting On Line Access will need to complete an On Line Access Registration Form that will need to be completed and submitted to the surgery in person together with one photographic form of ID (e.g. passport or driving licence) and one proof of address that is no more than 3 months old

Detailed Coded Records

Patients who have access to on line services can have the facility to view their detailed coded online record and they can apply for this via their on line access. This includes all information that is held on the practice's clinical system which is held in coded form unless any of the following apply:

In the reasonable opinion of the practice, access to such information would not be in the patient's best interest because it is likely to cause serious harm to:-

  • The patient's physical or mental health or
  • The physical or mental health of any other person
