We are aware many patients are anxious about the change in guidance from July 19th, and whether they should be wearing a face mask or not. We would like to commit to our continued efforts to safeguard our staff and patients and will be asking for the continued use of masks in all environments at the Practice, including the waiting room, reception, clinical spaces and administrative spaces. Covid-19 Vaccination Update

In line with the Government’s announcement, the NHS is aiming to offer every eligible person the chance to book a COVID-19 booster vaccine by the end of the year. This means our practice is now prioritising the booster rollout.

As part of this, some of our routine services may be stood down temporarily to allow us to focus on protecting as many patients as possible.

We are still there for anyone who may need care for an urgent need, but would ask for patience and respect as we support this key national priority over the coming weeks. We hope to be able to stand back up any services as soon as possible and will update this page as soon as we know more.

Please don’t forget there are many services available for you, depending on your condition. These include:

· Self-care – This is the best choice to treat minor illnesses and injuries such as hangovers, coughs, colds, grazes, small cuts, norovirus or sore throats. Many of these can be treated at home, simply with over-the-counter medicines and plenty of rest. · Pharmacy – Pharmacists can give advice on and treat a range of symptoms. They can support you with things such as diarrhoea, earaches, painful coughs, sticky eyes, teething and rashes.

· NHS 111 – You can access 111 online or via the telephone. This is for any urgent but non-life threatening health concerns. They will be able to signpost you to the relevant service and provide advice where appropriate or arrange for a healthcare professional to call you.

· A&E / 999 – You should only attend A&E or dial 999 in a life-threatening emergency such as severe bleeding, breathing difficulties and severe chest pain.

If you are now eligible for your COVID-19 booster please book it as soon as you can. It is vital as many people as possible get boosted as soon as possible. Please do not ring the practice regarding your booster, instead visit www.nhs.uk or by calling 119. 

If you are a housebound patient, there is a community team which will be coming out to you to vaccinate you, so please do not contact your GP as the team will be making contact with you.

Covid Vaccination Certificates

 We are unable to issue this certificate at the practice.

Proof of your vaccination status will be available on the NHSapp which is also valuable for accessing your health records and ordering repeat prescriptions

Alternatively you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.

To support promotion of the NHS App/COVID Pass, BT have produced a guide for those with limited digital skills. The campaign, endorsed by NHSE, is encouraging people to help others who might struggle with downloading and using the NHS App. The guide gives details on how to show vaccine status, ordering repeat prescriptions and how to access health advice.

Our dedicated team are here to treat those minor ailments that occur as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues. The technology also means you can now do a lot of things from the comfort of your home, such as order a repeat prescription or cancel an appointment.

Access
Several parking spaces are reserved for disabled persons in our main car park. Ramps for wheelchairs of disabled patients are sited at the main entrance. All consulting rooms and disabled toilets are designed to allow easy wheelchair access. Wolvey Surgery has been modified to comply with Disability requirements. Our premises at Burbage and at Wolvey are now covered by CCTV internally and externally for the safety of our patients and staff. Please see our Code of Practice below:

Since 31 July 2016, we have been required to comply with the Accessible Information Standard. The standard aims to make sure that patients who have a disability or sensory loss get information that they can access and understand, and any communication support that they need.

If you require a continuation sick note you can now email your request to the surgery using the attached continuation sick note request form. Please email your request to theburbage.surgery@nhs.net. This will then be sent to a doctor to authorise or advise if a doctor needs to speak to you. If a doctor needs to speak to you, a member of the reception team will contact you. During the current situation with Covid-19, all authorised continuation sick notes will be posted to patients for continued safety reasons.

If you wish to give consent to a 3rd Party to all us to be able to disclose information on your behalf regarding your medical conditions and health care you can now complete the form below to allow this access. Please use the attached pregnancy registration form to allow us to register your pregnancy with our Midwifery Team. This form is essential for all new pregnancies prior to patients being able to book an appointment with the Midwife.
