People most at risk from Covid invited for spring booster jabs

People aged 75 years and over, older people in care homes and those aged 12 years and over with a weakened immune system are being offered the spring booster. Covid is more serious in older people and those with a weakened immune system and their protection from the vaccine may be lower and may decline more quickly. The NHS is contacting those who are eligible, six months after their last dose - and people should wait until they are invited. If you are now eligible for your COVID-19 booster please book it as soon as you can. It is vital as many people as possible get boosted as soon as possible. Please do not ring the practice regarding your booster, instead visit www.nhs.uk or by calling 119.

Proof of your vaccination status will be available on the NHSapp which is also valuable for accessing your health records and ordering repeat prescriptions

Alternatively you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.
Patients are required to attend the appointment with a photo ID and evidence of their overseas vaccination.
To support promotion of the NHS App/COVID Pass, BT have produced a guide for those with limited digital skills. The campaign, endorsed by NHSE, is encouraging people to help others who might struggle with downloading and using the NHS App. The guide gives details on how to show vaccine status, ordering repeat prescriptions and how to access health advice.

Suspected Coronavirus -Important information to keep you safe while isolating at home
As part of the government's coordinated response to Coronarirus, NHS Test and Trace has developed the new NHS COVID-19 app. It is now available for download to all residents of England and Wales. Several parking spaces are reserved for disabled persons in our main car park. Ramps for wheelchairs of disabled patients are sited at the main entrance. All consulting rooms and disabled toilets are designed to allow easy wheelchair access. Wolvey Surgery has been modified to comply with Disability requirements. Patients can now self certify for 28 days. Please read the guidance below.
If you require a continuation sick note you can now email your request to the surgery using the attached continuation sick note request form. Please email your request to email@example.com. This will then be sent to a doctor to authorise or advise if a doctor needs to speak to you. If a doctor needs to speak to you, a member of the reception team will contact you. During the current situation with Covid-19, all authorised continuation sick notes will be posted to patients for continued safety reasons.
Consent/Permission to Disclose Data to a named 3rd Party
If you wish to give consent to a 3rd Party to all us to be able to disclose information on your behalf regarding your medical conditions and health care you can now complete the form below to allow this access. The form will need to be printed so that the patient who is giving permission can sign the request and then this can be emailed back to the surgery.

Please use the attached pregnancy registration form to allow us to register your pregnancy with our Midwifery Team. This form is essential for all new pregnancies prior to patients being able to book an appointment with the Midwife.

If you wish to use our Home Blood Pressure Monitoring Record then please use the form below and either email this back to us or send into the surgery. These results are monitored as they are received and added to your records and at this point an actions will be made if necessary.
